Eligibility

GAFC Services:

To be eligible for GAFC Services potential clients must:

  • Need daily assistance with personal care, home care, or help with daily tasks.
  • Are adults 22 years and older who meet the clinical criteria for the program.
  • Live in GAFC approved subsidized housing in Massachusetts.

Homemaking Services:

To be eligible for Homemaking Services a client must have one of the following insurance plans and be assessed for services by the insurance nurse:
  • Commonwealth Care Alliance
  • Tufts Health Unify
  • Senior Whole Health
  • BMC Health Net Plan
  • MassHealth Standard

Companionship Services:

To be eligible for Companionship Services a client must have one of the following insurance plans and be assessed for services by the insurance nurse:
  • Commonwealth Care Alliance
  • Tufts Health Unify
  • Senior Whole Health
  • BMC Health Net Plan
  • MassHealth Standard

Personal Care Services:

To be eligible for Personal Care Services a client must have one of the following insurance plans and be assessed for services by the insurance nurse:
  • Commonwealth Care Alliance
  • Tufts Health Unify
  • Senior Whole Health
  • BMC Health Net Plan
  • MassHealth Standard

Private Care Services:

A personalized individual care plan is created specifically for the private care client during an assessed meeting with a nurse and a case manager. To inquire about private care services from us please fill out referral form below or contact us directly here.
